Ibogaine has been associated with QT prolongation: a change in the heart’s electrical timing that can increase concern for dangerous rhythm disturbances, including torsades de pointes. The clinical meaning of a QT change depends on context, other health factors, and concurrent substances; it is not a question that can be settled by a general checklist.
Because of this signal, trial-style protocols commonly place substantial emphasis on cardiovascular history, baseline testing, and electrocardiographic observation. Cardiac screening and ECG monitoring are safeguards used in research-oriented settings; they do not make an unapproved substance predictable or risk-free.
Risk may be affected by pre-existing heart conditions, electrolyte abnormalities, dehydration, other drugs, dose, and acute changes during an episode. Research has also examined the pharmacology of noribogaine, an active metabolite that may persist after ibogaine exposure and can complicate the timing of safety observation.
